Clinical Supervisor – Home Health (In-Office with Field Capability)
Position Type: Full-Time

Position Summary
The Clinical Supervisor is a key member of the agency’s leadership team, responsible for overseeing day-to-day clinical operations, ensuring regulatory compliance, and supporting field staff. This role is primarily in-office, focused on clinical oversight, documentation review, and coordination of care. The Clinical Supervisor may perform skilled nursing visits in the field when necessary to support patient care continuity or staff coverage.
Key Responsibilities
Clinical Oversight & Compliance
Review and approve OASIS assessments, Plan of Care (POC), and visit documentation for accuracy and compliance with Medicare guidelines.
Monitor clinical workflows to ensure timely initiation of care, recertifications, and discharges.
Ensure adherence to CMS Conditions of Participation, PDGM requirements, and agency protocols.
Participate in QAPI activities and chart audits to maintain high standards of care.
Staff Supervision & Support
Provide clinical guidance and mentorship to field nurses, therapists, and home health aides.
Conduct case conferences and interdisciplinary team meetings to coordinate patient care.
Assist with onboarding, training, and competency evaluations of clinical staff.
Serve as a resource for EMR documentation, workflow questions, and regulatory updates.
Field Nursing (As Needed)
Perform skilled nursing visits (e.g., SOC, ROC, Recert, Wound Care) when coverage gaps arise or patient acuity requires supervisory intervention.
Maintain clinical proficiency and licensure to support field operations.
Communication & Coordination
Liaise with physicians, referral sources, and patients to ensure effective care coordination.
Collaborate with intake, scheduling, and billing teams to streamline operations and resolve issues.
Support survey readiness and respond to ADRs or payer audits as needed.
Qualifications
Registered Nurse (RN) licensed in [Insert State].
Minimum 2 years of home health experience; supervisory experience preferred.
Strong knowledge of Medicare home health regulations, OASIS, and PDGM.
Proficiency with EMR systems (e.g., WellSky, Homecare Homebase, Alora).
Excellent communication, leadership, and organizational skills.
