Description
Maximus is seeking a Registered Nurse, Clinical Assessor to join our growing team. As a Nurse Assessor, you will conduct person-centered assessments in accordance with contract directives, company processes, and contract requirements.
You’ll evaluate individuals in their homes, group homes, and care facilities to determine Long Term Care needs and appropriate placement options.
This position requires the ability to commute within and cover Waldo & Kennebec County.
About the program: Maximus has been contracted to partner with the State of Maine’s Department of Health and Human Services, through the Office of Aging and Disability (OADS), as manager of its Statewide Assessing Services. This single Assessing Services Agency (ASA) Program will encompass a series of programs, including Long Term Care (LTC), ABI, ORC, ICF/IDD, GPU.

Essential Duties and Responsibilities:
Complete medical and/or behavioral health assessments that are typically routine using standardized tools and criteria in accordance with contract requirements; escalate complex or non-standard cases that fall outside defined protocols
Conduct on-site, video, and/or telephonic assessments based on program requirements to determine an individual's needs for services and supports, eligibility, and level of care based on established guidelines, seeking guidance from senior staff as needed
Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies; ensure accurate and consistent documentation of assessment outcomes
Travel to client, consumer or program locations to perform in-person assessments exceeding 50% of the workweek in support of program delivery
Minimum Requirements:
Specific clinical license and experience requirements based on contract requirements
Travel on-site to client, consumer or program locations for more than 50% per workweek required; specific travel requirements based on program contract requirements
Minimum two (2) years of post graduate nursing experience
Preferred Requirements:
Experience in long term care or home health care
Experience with OASIS or MDS
